If you are a current patient at Portland Alternative Clinic renewing is easy. Just give us a call and we will review your records we have on file and set up an appointment. Keep in mind, depending on your qualifying condition, a medical visit to your primary care physician or specialist may be required to renew. Call if you have any questions.

If you are a Renewing Patient Who Used Another medical marijuana Clinic , you may request your records to be faxed or mailed directly to us from the other clinic’s office.

You can contact your physician’s office directly and request they submit your medical records to us either by medical release form they provide or you can download our medical release form and fax or mail it to them.

Depending on the Qualifying Condition and time of last visit, new records may be needed. Call us if you have any questions.

The State will only accept medical recommendations within 90 days of a physicians signature. Your responsibility is to ensure your office visit is within 90 days of your OMMP cards expiration date.

This will prevent another office visit to our clinic, as the State of Oregon will deny your application if the medical recommendation is greater than 90 days of the OMMP’s expiration date.

*Remember: The records need to be current within the past three years and must pertain to your qualifying condition. We only need records from 3 recent visits on your condition. These can be up to three years old.

*Records must be from an MD (medical doctor), DO (doctor of osteopathy), Nurse Practitioner (NP) or Physicians Assistant (PA).
